Civil
Case Intake Form			Amador Rule of Court 11.20 requires parties filing new cases, civil, family, probate, small claims, etc., to complete all identifying information available to them regarding all parties to the case and submit the form at the time of filing.
Stipulation to Mediator 04 17 08			Used for parties/counsel to stipulate to a civil mediator. For the list of the Court's Civil Mediators click here.

Family Law
Case Management Conference Statement			Used to inform the Court as to your updated Family Law case status.
Declaration re: Notice Upon Ex Parte Application for Orders			Used to give notice of ex parte hearing, or explain why no notice is being given.

Firearm Declaration			Used by restrained person to declare under penalty of perjury they do not have firearms, or they do and will be complying with the law and turning them in.

Criminal
1203.4 Instructions 1203.4 Forms			After successful completion of probation, you may petition the court to dismiss the charges under Penal Code §1203.4. If it is in the interest of justice, the judge may allow you to file the petition before probation ends. You may also petition the court under 1203.4a after conviction of a misdemeanor.
EX PARTE APPLICATION TO PLACE MATTER ON CALENDAR			Application is used to place matter on calendar when counsel/party requests a hearing date and is unable to comply with procedures.
ORDER ON EX PARTE APP TO PLACE ON CALENDAR			Proposed Order which must be submitted to the court in conjunction with the ex parte application to place matter on calendar.
APPLICATION TO SHORTEN TIME FOR SERVICE			Application is used to request court shorten time for service when counsel/party is unable to serve within the time limits set forth in the Rules of Court.
ORDER ON APPLICATION TO SHORTEN TIME FOR SERVICE			Proposed Order which must be submitted to the court in conjunction with the ex parte application to shorten time for service.
